Food Allergies / Dietary Restrictions*

Please list all food allergies and/or dietary restrictions (i.e. Vegetarian, Gluten Free, Dairy Free, keto, low carb, etc.), so we can work with you to see how best we can attempt to arrange meal plans for you. We can't guarantee all of our provided meals will avoid all of your food allergies or fit all of your dietary practices. But we will try our best to work together for solutions. (We are not a gluten free certified kitchen, there will be cross contamination and we will do our best). If you have no allergies or special dietary needs, simply answer: “None.”

Release, Waiver and Assumption of Risk*

This is a legally binding release, waiver, and assumption of risk made by me to IRONDEEP, LLC. (“IRONDEEP”). I wish for myself to be able to utilize the facilities hosted by IRONDEEP, as well as any off-site facilities where a particular activity is conducted during the retreat sponsored by IRONDEEP. For any retreat I attend, as well as to participate in any activities sponsored, organized or recommended by IRONDEEP, I hereby agree as follows: I acknowledge and understand that there are certain dangers and risks to which I may be exposed by utilizing facilities and properties hosted by IRONDEEP or participating in the activities, including the risk of serious physical injury, temporary or permanent disability, and death, as well as severe social and economical loss. The risks may arise not only from my own actions, inactions, or negligence, but also from the actions, inactions, or negligence of others, or the conditions of IRONDEEP hosted facilities/properties or of any equipment used. I also acknowledge and understand that there may be other risks not presently known or reasonably foreseeable. I understand that I am not required to participate in any activities or to utilize IRONDEEP hosted facilities/properties, but that I freely choose to do so, despite the possible dangers and risks and participation in the activities and utilization of IRONDEEP hosted facilities/properties.

By signing this form I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby. I hereby warrant that I am eighteen (18) years old or more and competent to contract in my own name.

Video/Audio Recording and Photography Waiver*

I hereby grant IronDeep, LLC. (“IronDeep”), its employees, agents, owners, officers and representatives the irrevocable right and permission to use video recordings and/or audio recordings and/or photographs of me, my image and likeness, in all forms and manner, including but not limited to, publication on IronDeep-owned and non-IronDeep-owned property, print media, digital media, the Internet, social media sites, media releases and broadcasts, events, and any other content, promotions or derivative works, without compensation or further notification to me.

I understand and agree that such photographs and/or video recordings and/or audio recordings of me may be electronically displayed online or in public settings. I also understand and agree that I may be identified by name and/or title in printed, Internet or broadcast information that might accompany the photographs and/or video recordings and/or audio recordings of me.  I waive the right to approve the final product. I agree that all such portraits, pictures, photographs, video and audio recordings, and any reproductions thereof, and all plates, negatives, recording tape and digital files are and shall remain the property of IronDeep. There is no time limit on the validity of this release nor is there any geographic limitation on where these materials may be distributed.

I hereby release, acquit and forever discharge IronDeep, and its owners, agents, officers, representatives and employees from any and all claims, demands, rights, promises, damages and liabilities arising out of or in connection with the use or distribution of said photographs and/or video recordings and/or audio recordings, including but not limited to any claims for invasion of privacy, appropriation of likeness or defamation.

By signing this form I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby. I hereby warrant that I am eighteen (18) years old or more and competent to contract in my own name.
